We are requested to complete a new patient questionnaire and a registration form and bring it with them to their first visit. You can click here to complete your forms online. As well, you will need to bring your insurance card and a picture identification card. You can print out the New Patient Questionnaire and Registration form from this website or ask that it be mailed to you if you wish.
We’ll be closed for the following holidays: Thanksgiving, Christmas Day, and New Year’s Day. We may have abbreviated hours on other holidays, please call ahead on other major holidays at 410-694-7999.
In the case of inclement weather please contact our office before coming to your appointment. We consider the safety of our patients and our staff, and evaluate the condition of the roads and the degree of icing of the parking lot before we decide on the hours that FirstCall Medical Center will be open each day. There is always a physician available by phone to care for emergencies even in the event of inclement weather.
When necessary, patients of FirstCall Medical Center are admitted by the physicians of the practice to Howard County General Hospital, a member of Johns Hopkins Medicine, in Columbia, MD.
Thank you for your adherence to these policies!
